        "Local NGOs responding to AIDS: a strategic planning workshop", represented the start of a second 'phase' of Alliance activities. It brought together representatives from 23 local NGOs with staff from government and international organizations to begin a process of identifying appropriate roles for local NGOs in an effective national response to HIV and AIDS, as well as the needs of the local NGO sector in fulfilling these roles. The workshop also aimed to assess if and how such needs might be met by a local NGO HIV/AIDS Support Programme which would serve as a local source of technical and financial support to a variety of community groups poised to respond to the epidemic. The main outcomes of this workshop, as well as issues which were raised, are summarized in this report, from the perspective of the International HIV/AIDS Alliance.

        This report assesses the current situation in the areas of population and development,reproductive health and gender, and proposes recommendations for strategic actions for addressing identified critical needs. The situational analysis has been guided by the International Conference on Population and Development - Programme of Action (ICPD-PoA). The ICPD recognised that population and development are inextricably linked and both depend on the empowerment of women. It focused on meeting the reproductive and sexual health needs of individual women, men and couples rather than on demographic targets. The analysis aims at establishing a framework for the development of the UNFPA-assisted country programme within` the context of the country's overall strategies and needs,as well as within LNTPA's mandate and comparative advantage:UNTPA's mandate is to assist developing countries, at their request, to improve reproductive health care and to promote sustainable development. Reproductive health care includes family planning, sexual health, information and counselling, and medical services, all on the basis of individual choice. The Fund also supports integration of population into development strategies, capacity building for generating and utilising data for policies and programmes, research and training on population and development.

        The Kingdom of Cambodia has the most serious HIV/AIDS epidemic in Asia with many contributing factors which suggest that the epidemic has the potential to cause Cambodia to become one of the worst affected countries in the world. The 1990s in Cambodia have been marked by an alarmingly rapid increase in the overall number of HIV infections and a rapid rise in the national HIV prevalence among several key populations. The first reported cases of HIV infection were in mid 1991 with unreported cases suggested to have begun in the late 1980s. A Comprehensive National Plan was developed in 1993 to guide the national response to the epidemic.

        .....All the women interviewed expressed the need for HIV and STI education and STI services. They preferred a quiet location, not too far from the workplace, so that they could access services quickly without paying too much for transport. Respondents reported that they preferred female educators and health practitioners who were middle-aged, competent and courteous. Based on this assessment, the research team has recommended that specific STI education and treatment services be made available to these women with an emphasis on ease of access, lack of discrimination and confidentiality.

        Cambodia's health status is among the poorest in the region, as Table 13.1 shows.  The public health system has suffered from war and chronic underfunding and is having serious difficulties in meeting the health needs of the population. A heavy burden of communicable diseases results in high mortality rates among children and adults.  Progress has been made in seeking to revitalize the health system and to break the vicious cycle of ill health, debt, and poverty that economically cripples Cambodian families and retards the country's development. However, considerable further effort is needed to improve access to health services of improved quality.         Cambodia's HIV prevalence rate is the highest in SouthEast Asia, and rising at the highest rate (Table 14.1). As noted in Chapter 4, HIV/AIDS does not simply kill. It affects all social classes, leads to long periods of incapacity, often strikes people in their most productive years, and demands enormous resources from carers. Moreover, HIV increases the likelihood of those infected with TB developing the disease. Given the prevalence of TB (Table 13.1), the HIV/AIDS epidemic will dramatically increase the TB burden. Avoidance of a disastrous epidemic requires swift preventative action.

        Despite the danger of a potential HIV/AIDS epidemic, Cambodia opted to procrastinate, believing international isolation would act as a shield, thus postponing the inevitable. As a result, since the first diagnosis of HIV in 1991, the infection rate has accelerated to one of the fastest growing in the world.Three key themes run through the chapters, looking at aspects of coordination. incremental policy-making,and the perception of AIDS as a health issue rather than a social problem. This latter focus has meant that the cost of the AIDS epidemic will be a burden increasingly carried by women, their children and will be borne disproportionately by the poor. One area largely ignored in Cambodia, yet pointed to in many international studies. involves the role of migrant workers. Whilst acknowledging the dangers of identifying "high-risk" groups, the dissertation assesses information on migrant workers and their families, and the impact they may have on the future spread of the virus. Material used within the penultimate chapter draws heavily on research, funded by the Dutch government, designed and implemented by the author during his course placement in Cambodia.

        The mission of the International HIV/AIDS Alliance is to support communities in developing countries to play a full and effective role in the global response to AIDS. In Cambodia, the Alliance is the parent organisation of KHANA, the Khmer HIV/AIDS NGO Alliance. Established in 1996, KHANA has spearheaded the community response to HIV/AIDS work in the country: to date, it has mobilised over 40 NGOs and is now recognised as the premier AIDS NGO support programme in Cambodia. KHANA also receives support from USAID, JICA, MoH - World Bank, AUSAID, EU, UNFPA (through the Reproductive Health Initiative for Asia) and the British Embassy.

        The HIV/AIDS epidemic in Cambodia is growing at an alarming rate, estimated in 1998 to have reached 2.8% of the adult population. The current uncertain political and economic situation suggests that factors fuelling the rapid spread will persist into the foreseeable future. The Ministry of Health initiated a plan to control the epidemic in 1991, with their efforts supported by an increasing number of bi-lateral donors, UN Organisations and NGOs. One recent initiative has been the drafting of the National Strategic Plan 1998-2000, which aims to be a 3 year framework for action and coordination of the response to HIV/AIDS. This document identifies AIDS Care as a priority area, and discussion has begun in an attempt to address this.  However, it remains as a significant gap in the resource allocations of the major players, both government and non-government.

        This study projected costs of scaling up government health services to achieve improved health status of the poor and undeserved segments of the Cambodia population.  

Three main types of interventions were explored: two alternative interventions for improving the quality of health services, three interventions that improve access to services by the poor, and twelve that target health problems that burden the poor relatively more.

Total and marginal unit costs per capita were estimated by reviewing existing studies of interventions in Cambodia and in other countries with similar health and socioeconomic environments.  Where it was relevant, previous efforts to cost interventions and programs were reviewed in detail and some data extrapolated from these.

        Under this Strategic Plan, the National Centre for HIV/AIDS, Dermatology and STDs (NCHADS) developed a series of specific Policies (eg for Testing, for STDs, for Blood Safety, etc.), Strategies (eg. for Surveillance, for AIDS Care, for Outreach, etc), Guidelines for the Introduction and Implementation of various programmes and interventions (eg. I00% Condom Use, Home-based Care), and Training packages (eg. Syndromic Management of STDs, Strengthening Provincial HIV/AIDS Programmes, etc).         This paper described MSF and its partners intentions in establishing, as clearly as possible, a general policy on AIDS, common to all MSF sections. Basically, it aims to define a general framework for our interventions. It is recant particularly for the people in the field, and should be practical enough to say what has to he done and what should be avoided. MSF works in different types of situations with different opportunities or limitations in actions.  Therefore this policy document will outline the possibilities of action within MSF projects mid also define a minimal response required in all situations.         The Government of Cambodia realizes that HIV/AIDS is an urgent priority because this disease is causing an increasing public health, economic and social problem for the whole country. The main route of transmission of HIV/AIDS is occuring through sexual intercourse between men and women. Men play the bridging role by infecting their wives, babies and other partners where they get from HIV infected women. Since HIV/AIDS has transmitted in an alarming rate, the main strategy to response to the epidemic is to prevent people from getting infected because so far there is no cure for this disease. The 100% condom use is promulgated as a national policy and shall be applied in all brothels and sex establishment to ensure the safety of sex services.

        The current HIV/AIDS epidemic in Cambodia is one of the most serious situations in Asia.  Since the detection of the first case of HIV infection in 1991, the disease has spread very rapidly throughout the country, as witnessed by the very high rates of HIV infection observed in many groups of population, even in groups at low risk such as pregnant women.

The main mode of HIV transmission in Cambodia is heterosexual contact between sex workers and their clients who further spread the disease to housewives and the newborn.

Although many interventions are implemented to prevent heterosexual transmission of HIV in Cambodia, the rapid spread still continues to increase. The effective control of HIV seems to rely on the prevention of HIV spreading between sex workers and their clients. Thus it is recommended that the policy to promote 100% condom use in sex establishments should be
considered for implementation in Cambodia. It is feasible to conduct the 100% condom-use project in one province of Cambodia, the results of which can be use for nationwide expansion of the policy.

        Like many other countries in South East Asia, Cambodia is experiencing a rapidly developing AIDS epidemic. Groups reported as being particularly seriously affected include sex workers and their clients. Young people too may be at heightened risk: some young women find sex work a lucrative option in the context of low wages and poor employment opportunities, and some young men pay for sex either as individually or as part of group socializing. These same young men may subsequently have sex with other partners, thus extending networks of transmission. While there is limited knowledge about the form of such sexual networks, little is known about the meanings that underpin young people's sexual relations and partnerships, sexual identities associated with such meanings, and prevailing socio-sexual cultures. This paper reports on findings from an indepth qualitative study conducted among two groups of young people: one urban, the other rural. Following an initial Rapid Assessment Process, .data was collected via individual interviews, focus group interviews and participant observation. The research team included. young people themselves Data is presented on dominant discourses about sex and sexuality in Cambodia; contemporary patterns of sexual behaviour; sexual meanings and sexual practices sexual relations.
