        This study was commissioned by WHO/Cambodia to measure the costs of the Minimum and Complementary Package of Activities. Costs of services at provincial hospitals, ODO referral hospitals and Health Centers were measured over three months in 2002.         This study projected costs of scaling up government health services to achieve improved health status of the poor and undeserved segments of the Cambodia population.  

Three main types of interventions were explored: two alternative interventions for improving the quality of health services, three interventions that improve access to services by the poor, and twelve that target health problems that burden the poor relatively more.

Total and marginal unit costs per capita were estimated by reviewing existing studies of interventions in Cambodia and in other countries with similar health and socioeconomic environments.  Where it was relevant, previous efforts to cost interventions and programs were reviewed in detail and some data extrapolated from these.

        Under this Strategic Plan, the National Centre for HIV/AIDS, Dermatology and STDs (NCHADS) developed a series of specific Policies (eg for Testing, for STDs, for Blood Safety, etc.), Strategies (eg. for Surveillance, for AIDS Care, for Outreach, etc), Guidelines for the Introduction and Implementation of various programmes and interventions (eg. I00% Condom Use, Home-based Care), and Training packages (eg. Syndromic Management of STDs, Strengthening Provincial HIV/AIDS Programmes, etc). These have all been used to establish activities and programmes both at Central and Provincial level.

        The Government of Cambodia realizes that HIV/AIDS is an urgent priority because this disease is causing an increasing public health, economic and social problem for the whole country. The main route of transmission of HIV/AIDS is occuring through sexual intercourse between men and women. Men play the bridging role by infecting their wives, babies and other partners where they get from HIV infected women. Since HIV/AIDS has transmitted in an alarming rate, the main strategy to response to the epidemic is to prevent people from getting infected because so far there is no cure for this disease. The 100% condom use is promulgated as a national policy and shall be applied in all brothels and sex establishment to ensure the safety of sex services.

        The National Strategy on Immunization in Cambodia contains objectives for
routine immunization coverage, polio eradication, neonatal tetanus elimination, measles control, vitamin A deficiency, new interventions, injection safety, and vaccine wastage.

        This is a national strategy which provides broad guidelines that should be used by all sectors of the Ministry of Health and other related ministries, in particular the Ministries of Women's Affairs and Rural Development, International Organisations, bilateral agencies and NGOs. This is to ensure that Safe Motherhood does not become a vertical programme but that all its components are integrated into existing Maternal and Child Health/Birth Spacing (MCH/BS) and related programmes.

The main focus of the policy will be on improving maternity care services including child spacing and nutrition at all levels of the health care delivery system starting from the family and the community. It will also aim at behavioural and societal changes at community and service delivery levels in order to improve community participation.

        On 28 December 1996 His Majesty Norodom Sihanouk, King of Cambodia called and chaired a three hour working session on the issue of Child Rights which was attended by ministers of Education, Social Affairs and Industry and some parliamentarians. His Majesty stressed the need for additional legislation to reflect the Convention on the Rights of the Child in Cambodia's laws and decrees and expressed his active support for programmes working towards the elimination of hazardous and exploitative child labour.

        The survey showed some important differences in public health facility utilization between ADD and non-ADD areas. This probably reflects, however, the selection of ADD areas, many of which were chosen due to existing facilities, and human and material resources. It is inconclusive, therefore, that the provision of technical equipment and materials alone is sufficient to significantly increase utilization in public health facilities.

        One of the conclusions of the study is that there is potential to improve the HCP by incorporating providers' sense of roles and responsibilities, especially as they relate to the community. Secondly the 'manoeuvring' in the health centre is a complex response that is not motivated by economic goals alone. However it is clear that manoeuvring will continue while ever the mismatch between expectations of the staff and the resources available to them continues. In addition there is evidence that manoeuvring is representative of strong and creative leadership in many instances, and that there is substantial capacity at the health centre level to develop management skills. The current environment however is resistant to decentralisation and there is little experience of decision making responsibilities being held at lower levels such as the OD and the health centres. And finally this paper concludes that regardless of the strategies employed by donor agencies and the MoH, health providers will support reform to the extent that reform is congruent with provider's needs and their professional roles
